Project Description

To reduce exposure to COVID-19 in congregate settings and to treat mental health and substance use disorder issues that have been exacerbated by the pandemic, this project will develop and launch a technology solution to support the delivery of a single, cohesive, integrated pathway to individuals (Clients) who come into contact with the justice system in order to get these clients engaged in treatment and services that address the underlying reasons that resulted in their justice system contact as soon as possible. The goal is to develop a web-based mobile application that will facilitate clients’ ability to obtain mental health and substance use treatment as well as other services in order to divert eligible Clients who encounter the justice system and link them to the appropriate services and/or treatment including placement in an appropriate service bed. It will also be available to individuals who are not involved with the justice system, and will help prevent involvement from occurring. Such services will include: Care management and service navigation Basic needs Housing Employment and education Crisis stabilization Acute/subacute inpatient Crisis/extended residential Outpatient care + housing Correctional health levels of need During the COVID 19 Pandemic many of the underlying issues affecting these individuals were exacerbated and made even worse. This can potentially lead to a rise in situations that may require assistance and aid of all kinds. This technology solution must provide capability and functionality to conduct the Client’s needs assessment, to facilitate real-time referrals and schedule service appointments to available service providers. It shall support many types of users whether they are Clients, Service Navigators, Service Providers, Care Coordinators, Judges, or Court Personnel.

Goals & Objectives

Goal Description A web-based mobile service navigator app will be developed by 2024 in order to connect at least 100 individuals in need with programs and services.

Objective Description The app will be designed through consultation with providers, navigators, and individuals with lived experience to ensure app useability/user-friendliness.

Activity Description The app will be developed and tested through a contracted developer who will lead focus groups and testing phases. The app will be launched and marketed through intensive outreach measures.
